You will be asked to show proof of your ID in the form of photographic identification and a formal letter showing your current address (for example, a utility bill dated within the last three months) –  all ID will be securely destroyed to keep in line with GDPR.

Complete Our Registration Forms

If you are already a patient and are about to move to a new address please let the patient service team know by completing our change of details form. They can decide whether or not you are still within our catchment area. If you are not, and you would like to stay with the practice, please let us know. All cases will be dealt with on an individual basis, by the practice manager and your named doctor.

All form submissions will be checked by a member of staff before being accepted.

Other Ways to Register

For identification purposes, you will need to upload proof of ID and proof of address at the end of the form. Proof of ID can be a passport or driving licence. Proof of address can be a utility bill (gas, electricity or water) or bank statement – dated within the last 3 months.

All registration applications must be submitted with your NHS number, applications submitted without NHS number may be rejected. If you are unsure whether you have this, please refer to this guidance.

Temporary Services

If you are ill while away from home or if you are not registered with a Doctor but need to see one, you can receive emergency treatment from the local GP Practice for 14 days if you register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local Practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that Practice.

To register as a temporary patient simply contact the practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a Practice in the town or area where you are already registered.

Your Named GP

Springhead Health Ltd. provide all patients with a named 'usual GP' who will have overall responsibility for the care and support that our surgery provides to them. We encourage patients to try and see their usual GP where possible, particularly for ongoing problems.  

We need to divide our patients evenly between all of our doctors, so they can manage their workload. Therefore the 'registering GP' rotates from week to week based on each GP's list size. We try to keep families under the same usual GP where possible, so new babies will be registered with their mother's usual GP.

If you wish to be told the name of your usual GP, please ask the reception team when you are next in the surgery.
